1.SCOPEANDOBJECTIVES
1.1.Scope
Thebookisconcernedwiththeoreticalandmethodologicalfoundationsof
clinicaldecisionsupportsystems(CDSSs)iACDSSis―anyprogramdesigned
tohelphealthcareprofessionalsmakeclinicaldecisions‖[131].Thisbroaddefi-
nitioncoversseveralcategoriesofcomputer-basedsystems,including:
systemsformanaginginformation,
systemsformonitoringandfocusingattention,
systemsforprovidingpatient-specificrecommendations.
Systemsformanaginginformation(e.g.,electronichealthrecords,EHRs)and
formonitoringandfocusingattentiononcriticalinformation(e.g.,vitalsigns
monitors)havebeenusedinclinicalpracticeforalongtimeandarewellac-
cepted[7,149].Interestinsystemsprovidingpatient-specificrecommendations
(patient-specificCDSSsinshort)isincreasing,inparallelwithdemandstoim-
provehealthcareprocessesandpatientoutcomesinacost-efficientmanner[89].
Areviewofclinicaltrialsofdiversifieddecisionsupporttools[94],including
paper-basedandcomputer-basedones,listedcomputer-basedimplementation
andtheiravailabilityatthepointofcare(whenandwherenecessaryduring
apatient-physicianencounter)asthemajorfactorsofsuccessinimprovingclini-
caloutcomes.Despitethesefindingsonly40%ofCDSSsreviewedin[19]were
availableatthepointofcare.
Thisbookfocusesonpatient-specificCDSSsavailableatthepointofcare.It
addressestheoreticalandmethodologicalaspectsoftheirdesignanddiscusses
multidisciplinarytopicsfromthefieldofmedicalinformaticscombiningsys-
tem3sanalysisanddesign,knowledgediscoveryandinformationretrieval.The
ultimateaccomplishmentofthisworkisanovelCDSSdesignandnewmetho-
dologicalsolutionsforconstructingdiagnosticdecisionmodelsfromclinical
dataandforindexingandretrievingclinicalevidencerelevantforthepatient-
-physicianencounter.
Theresultsofthedescribedresearchhavesoundtheoreticalandmethodolog-
icalfoundations,andatthesametimetheyhavebeenverifiedpracticallyinpilot
implementationsandaclinicaltrial.Theymayhavesignificantimpactonclini-
calpractice.Providingcomputerizeddecisionsupportatthepointofcareisim-
portantforthephysicianandforthepatient:fromthepatient3sperspectiveitim-
provesthecareoutcomesandthusthequalityofcare;fromthephysician3spers-
pectiveitimproveseffectivenessofdecisionmaking.
