Abstract:Patentforamenovale(PFO)representsthemostcommonpersistent
abnormalityoffetalorigin.Theaimofthestudywastoanalyzetheincidence
andcharacteristicsofPFOandtoassessPFOastheriskfactorofparadoxical
embolism.1920consecutivetransesophagealechocardiograms(TEE)inpts
examinedattheperiod1998-2010(902Fand1018Maged13–86,mean
52.95±12.21years)inwhomflowthroughPFOwasdetectedinbasalconditions
(color-Doppler)and/orduringprovocation(Valsalva/coughmaneuver)with
contrastwereretrospectivelyanalyzed.377ptshadahistoryofsystemic
embolism(SE)ortransientischemicattack(TIA)–GroupI,1543ptshad
negativehistoryofSE/TIA–GroupII.Twosubgroupsofptswere
distinguished:ptswithnoabnormalitiesontransthoracicechocardiography
(TTE)andnohistoryofcardiovasculardisease(CVD)–GroupI-A+II-Aand
ptswithabnormalitiesonTTEorpositivehistoryofCVD–GroupI-B+II-B.
PFOwasdiagnosed(PFO/+/)in157pts(8.2%)-60F,97Mmeanage52.65
±12.55years.In1763ptsPFOwasexcluded[PFO(-)].Aneurismalformationof
interatrialseptumwasdetectedin42pts,andin13ptsitcoexistedwithPFO.
TheintensityofshuntwassignificantlyhigherinI-B+II-Bgroupthan
inI-A+II-Agroup.In10ptspermanentshuntthroughPFOatthebasalstage
wasobserved.PercentageofptswiththehistoryofSE/TIAwassignificantly
higheringroupPFO(+)I-AthaninPFO(-)I-A(p<0.01).
Conclusions:1.InptswithnoabnormalitiesonTTEandnohistoryofCVD
PFOisapossibleriskfactorofSE/TIA2.FlowthroughPFOisgenerally
minimal,butincaseofcoexistingheartpathologyPFOmayprovokesignificant
right-to-leftorevenleft-to-rightshunt.
