kaniezgłaszaładolegliwościwynikających

językawynikającezumiejscowieniaśruby
aparatu,atymsamymutrudnionemówienie
ipodrażnieniejęzyka.Kluczowąrolęodegrała
wtymprzypadkusilnamotywacjapacjentki.
Pozablokowaniuśrubyaparatpozostawio-
nona3miesiące.PozdjęciuaparatuHyrax,
wcelustabilizacjiuzyskanejszerokościłuku
bypatient’smotivation.Afterthescrewwas
blocked,theappliancewasleftfor3months.
AftertheHyraxappliancewasremoved,an
individuallyadjustedpalatalbarwithpas-
sivearmsadjacenttopalatalpremolarswas
bent.Permanentappliancewasplacedinthe
maxillainorderto:
•aligntheteeth,
•re-createspacefortooth22bytheuseof
acoil-spring,
•subsequentlyfollowedbyplacingthetooth
backintothedentalarch.
Inthelowerarch,thebraceswerebonded
aftersixmonths.Afterproperrelationsin
botharcheswasachieved,a3monthsperiod
wasleftforretention.Intotal,thetreatment
lasted26months.Afterthebracesinupper
archwereremoved,aretentionplatewith
mesialscrewandlabialarchwithacrylic
sheathadjacenttotheincisorswascreated.
Asforthelowerretaineritwasbondedfrom
caninetocanine.Duetoadifferentshapeand
sizeofthecentralmaxillaryincisors,tooth21
wasre-contoured.
Asaresultoftreatment,animprovement
inchewingwasachieved,aswellcorrection
ofpatient’spronunciation–bythemeansof
speechtherapy–andlastbutnotleast:sat-
isfactoryaestheticeffects(Fig.4a-c,Fig.5).
Arapidmaxillaryexpansionwithoutsur-
gicalassistanceturnedouttobeasuccessful
methodoftreatingbilateralcross-biteand
asevereteethcrowdinginananteriorseg-
mentofmaxillaina20yearoldpatient.
Theinter-caninewidthincreasedby8mm
reaching32mmafterexpansion.Inter-molar
widthexpandedby4mmintotal.Condition
after11monthsoftreatmentisstable.
